Image injection neuronavigation using continuous tracking of the patient and microscope during skull base surgery

Abstract
Objectives: We herein report our experience with image-injection neuronavigation using real time tracking of patient and microscope with LED's attached to a locking acrylic dental stent (LADS). Methods: LADS are constructed with an LED array of known configuration. Precise registration was achieved by acquisition of CT and MR data sets after the insertion bone fiducials. During surgery, head and microscope movements were detected by an Optotrak. Segmented image data were injected into the oculars of a Leica microscope to give a 3-D effect. Results: LADS did not interfere with anaesthesia or surgery. Re-insertion experiments showed registration errors of lmm. Intra-operative overlays of bone fiducials and relevant structures were accurate to ~7E-±1.5mm. No complications were encountered and the overlays provided useful information. Conclusions: Use of the LADS to track head movement throughout surgery was simple and accurate. It is likely to be most useful and suitable for dentate patients undergoing complex anterior craniofacial resections. Injection of segmented overlays into the operating microscope appears to be superior to currently available displays.
