A Treatment Planning the Comparison of Scanned Proton Beams Versus Scattered Proton Beams

Abstract
Purpose: Radiation therapy is the backbone for the treatment of many cancer cases. An alternative to photon therapy is proton beam therapy, which is becoming increasingly well-known and is being used to treat certain types of tumour. It can be delivered to target with a minimum entrance dose and no exit dose, which is considered as one of the key advantages of proton over photon treatment. Clinically, proton beam may be delivered by two main methods; passive scattering and active scanning. This project will make comparisons between scattered and scanned treatment plans using proton beam therapy for two brain tumour cases in order to investigate the merits and limitations of each delivery system in covering target volume and sparing normal tissue. Methods and Materials: Two brain cases one is glioma and another is meningioma were planned by same commercial treatment planning system according to UCLH protocols. Each case has three plans: two scanning beams, three scanning beams and scattering beams. Result: Regarding the 3D gamma evaluation (within the ±2%/ ±2 mm gamma criterion), dose-volume histogram and dose homogeneity index, the similarity between outcomes of the two and three scanning beams is very high. However, the main difference between the scanning and scattering is in the target coverage which is better in both scanning plans. Conclusion: Both proton therapy delivery techniques can produce dose distributions within clinically accepted tolerance. However, two scanning beams can deliver the required dose and evenly distributed within the target volume as well.
